Who we are
Restore OT is a private occupational therapy practice based in Grantham, Lincolnshire, led by Edith Odunuga, an HCPC registered occupational therapist and member of the Royal College of Occupational Therapists.
For data protection purposes, Restore OT is the data controller for information you share with us. You can reach us at edith@restoreot.co.uk.
What we collect through the contact form
The enquiry form asks for your name, email address and, optionally, a phone number, your preferred contact method, the type of session you are interested in, and a short message describing what you need help with.
You may also list any documents you would like to share (for example a discharge summary or equipment list). The form itself does not upload files — you send those by email once we reply, so they travel directly between you and us.
Why we use it
We use your details only to respond to your enquiry, arrange an assessment, and provide occupational therapy services you ask for. Our lawful basis is our legitimate interest in responding to enquiries, and the performance of a contract once you become a client.
Where you share health information, we treat it as special category data and rely on the provision of health care and treatment by a registered professional bound by professional confidentiality.
Please keep clinical detail out of the form
The website form sends your message via your own email application. Please keep it brief and avoid detailed medical information — we will gather that securely with you directly, by phone or in person.
How long we keep it
Enquiries that do not lead to an assessment are deleted within 12 months. Clinical records for adults are retained for 8 years after the last contact, in line with professional record-keeping standards, then securely destroyed.
Who we share it with
We never sell your data or use it for marketing. We share information only with your consent — for example with your GP, consultant, case manager or family — or where we are required to by law, or where there is a serious risk to someone's safety.
Cookies and this website
This site uses only essential cookies and browser local storage: one entry remembers your cookie choice, and another remembers progress in the self-check questionnaire so it is not lost if you refresh.
The self-check questionnaire is anonymous. Answers stay in your browser and are never sent to us or to anyone else. We do not use advertising, profiling or third-party tracking cookies. You can clear this data at any time in your browser settings.
Your rights
You have the right to ask for a copy of the information we hold about you, to have it corrected or erased, to restrict or object to how we use it, and to withdraw consent at any time. Email edith@restoreot.co.uk and we will respond within one month.
If you are unhappy with how we have handled your information you can complain to the Information Commissioner's Office at ico.org.uk.
